  • Relevant health and mental health history

  • Details of the services provided to you

  • Notes from therapy sessions

  • Provide and coordinate therapeutic services

  • Assess and monitor your progress

  • Contact you for appointments or follow-ups

  • Communicate with other professionals only with your consent (e.g., doctors or insurance providers)

  • Meet legal and professional obligations

By engaging in therapy at Men’s Wellness Clinic, you are providing implied consent for the collection, use, and disclosure of your personal information for the purposes listed above. We will seek explicit consent for any uses beyond these purposes or where required by law.

You may withdraw your consent at any time, except where doing so would prevent us from fulfilling legal or professional responsibilities.

4. Confidentiality and Disclosure

Your personal information is kept confidential, except in the following situations where disclosure is required or permitted by law:

  • If there is a risk of harm to yourself or others

  • If there is suspected abuse or neglect of a child or vulnerable adult

  • If records are subpoenaed by a court

  • If required by a regulatory body such as the OCSWSSW for professional conduct review

  • Limiting access to only authorized staff and professionals

We retain your records in accordance with PHIPA and OCSWSSW standards—typically for a minimum of 10 years after your last appointment (or 10 years after you turn 18, if you were a minor during treatment).

6. Accessing and Correcting Your Information

You have the right to:

  • Access your personal health records

  • Request corrections to inaccurate or incomplete information
